General Summary:
Compass Medical is one of the largest groups of primary care providers with some 90 Family and Internal Medicine providers within 6 office sites across southeastern Massachusetts.  Affiliated with Steward Health Care System, Compass has an outstanding reputation for high quality care outcomes and use of information technology.  The Chief Medical Officer is seeking a Community Resources Specialist to assist patients with referral for specialized services such as community-based treatment, entitlement programs, transportation services, and general outreach to and coordination with other community resources and agencies.  The successful candidate will have a Master's Degree in Social Work (MSW).  He/she will be a resourceful, take-charge problem solver who can work independently and collaboratively.  He/she will be an empathetic, mission-driven, caring individual who goes the extra mile in helping patients, their caregivers and family members resolve complicated issues.  Excellent interpersonal communication skills are essential as this position requires frequent telephonic and face-to-face interaction with physicians, site managers, hospital-based case managers, patients, and family members.  The position requires the candidate to have a strong working knowledge of Microsoft Word and Excel for preparation of reports and data analyses and experience with electronic medical records and patient registry tools.

Principal Duties:
1. Identify entitlement programs and community-based agencies and resources relevant to Compass Medical patients with transportation, housing, and other needs.
2. Assist Compass Medical nurses and nurse care managers with completing applications, referrals, and other administrative requirements necessary for patients to receive entitlement program and community-based services.
3. Meet with patients and family members in office or at home as needed to complete assessments, education and advocacy.
4. Data entry into electronic medical record for documentation and communication purposes.

Minimum Requirements:
1. Master's Degree in Social Work.
2. At least two years social work experience preferably within outpatient clinic or medical group practice setting.
3. Proficient use of Microsoft Word, Microsoft Excel, patient registries, and web-based tools.
4. Ability to travel to Compass sites, hospitals, SNFs, rehabilitation facilities and patients' homes.
